If the event includes an acupuncture portion, you will receive a New Appointment email from Grand Madison Acupuncture with a link to complete New Patient forms online. Although you will be required to fill out some basic fields, you may skip the rest and continue to the Consent Forms at the end.

Please be sure to complete these forms prior to arriving at the event, or plan to arrive at least 10 minutes early to complete at the event.
